The Group you have been taking part in as part of the SKILL Project is supported with funding from the Australian Government Department of Social Services (DSS). DSS have asked us to ask you about your experience with SKILL and what you think you may have gained by taking part.
Things you should know
- This is your opportunity to tell us what you think about SKILL. We are asking everyone who has joined us the same questions. It is your choice. If you do not want to fill in this form, you do not have to.
- We will put all the answers we get together and give them to the DSS so they can also know what people thought was good or not good for them about SKILL.
- Everything you say is private. We will not give your name to the DSS or anyone else.
How to fill in the form
- Some questions will ask you to mark a box from a list. Please tick one box only. Some questions also have pictures. You can mark the picture instead of the words if you prefer.
- Some questions please answer in your own words.
- You can ask someone if you need help or you can skip any questions you do not understand.
- We will not ask you to give your name. We will not give your name to DSS or the National Disability Insurance Agency.
